Operating Budget Requests (Divisions, Roundtables, Committees)

Divisions Chairs, Roundtable Moderators, and Committee Chairs are to use this form to request an operating budget each year. The form is submitted to the Association Administrator and Treasurer. Submission of the form does not guarantee the funds requests will be approved. Each request must be submitted to the Treasurer via the Association Administrator on the C4: Operating Budget Request Form (Divisions, Roundtables, Committees) (available in both Word and PDF).
